Mallory - Weiss Syndrome is..
Smaller tears in the lower esophagus and upper stomach
How can you get MW Syndrome?
Violent Retching
What is Boerhaave’s syndrome?
You retch so hard you tear your esophagus
What happens to food in boerhaave’s syndrome?
Goes through esophagus into mediumstinum
Causes of both B and MW syndromes?
Alcohol use
ASA use
Heavy lifting
Pregnancy
Both B and MW have what clinical sign
Hematemsis
Additional symptoms of Borehaave r/t pain?
Excruciating retrosternal / upper abdominal pain due to acid in your chest
Borehaave swallowing?
Odynophagia
Borehaave breathing?
Tachypnea / Dyspnea
subC empyhsema
Tx for both mallory-weiss
Antiemetics - Treat Shock
Endoscopy w/ cauterization
Tx for Boerhaave’s syndrome

Esophageal Varices what are they??
Blood that leaves the heart and goes down the blood vessels of the esophagus into the liver
Liver swells and backs up blood into the esophageal blood vessels
This swells the esophagus - when food enters it scrapes it and causes ruptures - every time the heart beats it causes blood to leak into the GI tract
Splanchnic Vasoconstrictors (mostly used)
Somatostatin/Octreotide
Other splanchnic vasoconstrictors
Vasopressin - but can cause serious hypotension
Vasoconstrictor that can be used for varices but has more complications
Vasopressin (but causes systemic hypertension)
May need to give with nitro to allow cardiac vasodilation
