Will you have dehydration / Hypovolemia?
Yes - 100% of the fluid in your body is absorbed in your bowels - if it’s blocked you can’t get it
Problems that occur from bowel obstructions
Bowel Ischemia
Bowel Perf
Small Bowel Onset?
Rapid
Large Bowel Onset?
Gradual
Small Bowel Pain?
Wave-Like Cramp
Large Bowel Pain?
Low- grade
Abd Pain
Small Bowel Vomit?
Frequent and copious (Bile and feces)
Large Bowel Vomit?
Rare ( doesn’t tend to back up into stomach)
BM for small bowel?
Feces for a short period of time
BM for large bowel?
Absolute constipation (no gas)
Small Bowel adb distension?
Minimal increased
Throw up the “stuff” - so does not tend to distend
Large Bowel abd distension?
Yes because they are retaining everything
BOTH large and small bowel obstructions have what type of BP?
Hypertension early due to pain
Hypotension late due to hypovolemia
What auscultation can you hear with small and large bowel obstructions?
OVER SITE OF OBSTRUCTION : Borborygmi - Gurgling/Tingling as stool and water mix and hit the obstruction
Late sign: Absent bowel sound
WBC? HR? Fever?
High WBC
Tachycardia
High Fever
Pyloric Stenosis is predominately in
Pediatrics - shortly after birth (3-12 weeks)
Pylori muscle continues to grow and obstructs the outlet
Obstruction between the stomach and the duodenum
Pyloric stenosis vomit?
Projectile vomit
Pyloric stenosis nutrition?
Failure to thrive
Few stools
Dehydration
What is pyloric stenosis
Hypertrophy and hyperplasia of the pyloric muscle and narrowing of the gastric antrum
What you may be able to see with pyloric stenosis?
Peristaltic waves from right to left
What may you be able to palpate with pyloric stenosis?
Mobile, hard shaped olive mass over the pylorus
Intussusception is common in..
ages 3mo to 5 years of age
Intussusception is when..
The cecum (stronger part of the bowel) telescopes into the ileum (weaker part of the bowel)
Intussusception w/ blood vessels?
When the ileum telescopes inside the cecum it cause bowel to become ischemic due to kinking of BV - will begin to bleed - mixes w/ blood and mucous
Pain w/ intussusception
Painful, children may pull legs up to their stomach
Stools in intussusception
Grape jelly like (gangrene)
Tx for bowel obstructions
IV fluids / Electrolyte Imbalances Antiemetics Analgesics Rest the bowel Gastric Tube
Intussusception care
Barium/Air Enema
Weight of barium will reduce it
Pyloric Stenosis and perf care
Surgery
