Main Driveshaft Failure
Indications:
- Left Yaw
- Rapid Nr decay
- RPM warning light and aural tone
- High Np
- Zero Torque Indication
- Grinding sounds with possible loud banging noise
Procedures:
- *Autorotaion - ACCOMPLISH
CBOX Oil Overtemperature
Indications:
- *CBOX TEMP HI caution messsage and aural tone
- CBOX oil temperature guage indicates high
Procedures:
- *COOLER PBA - Select other position
MRGB Oil Overtemperature
Indications:
- MRGB TEMP HI message and aural tone
- MRGB oil temperature indication high
Procedures:
- *COOLER PBA - Select other position
Jammed/ Fixed Pitch/ Loss of Tail Rotor Pitch Control
Indications:
- Directional pedals do not move, or if they do, do not effect a change in yaw
- Yaw movement related to changes in collective
- Aircraft may experience sudden excursions in pitch, roll, and/or yaw
Procedures:
- *Controls - ADJUST (Use power and airspeed to align the nose).
Complete Loss of tail Rotor Thrust/ Tail Rotor Components (while in Forward Flight)
Indications:
- Right Yaw
- Possible nose tuck
- High requency vibrations
Procedures:
- *Autorotation - ACCOMPLISH
Complete Loss of Tail Rotor Thrust/ Tail Rotor Components (While in a Hover)
Indications:
- Right Yaw
- Possible nose tuck
- High Frequency vibrations
Procedures:
- *Throttles - IDLE
- *Autorotation - ACCOMPLISH
Fuselage Fire In Flight
Indications:
- Smoke
- Fumes
- Fire
Procedures:
- *Land Immediately
Engine-Driven Suction Fuel Pump Failure
Indications:
- ENG 1 or 2 BST PUMP FAIL cuation message with an aural tone
- Engine performance indications decrease (affected engine)
- Torque indication for non- affected engine will rise
- Nr decreases
Procedures:
- *XFEED PBA - OPEN (illuminated)
Autorotative Landing
Procedures:
- *Controls - ADJUST (to maintain Nr and desired airspeed)
- *Stores - JETTISON (as appropriate)
- *Throttles - CLOSE
Rotor Brake Pressurizes In Flight
Indications:
- ROTOR BRAKE warning light and aural tone
- ROTOR BRAKE voice message
- Rotor brake handle not stowed
- Decrease in Nr
- RPM warning light and aural tone
Procedures:
- *Rotor brake handle - STOWED
If warning light remains illuminated and seconday indications exist
- *HYD 1 PBA - OFF (illuminated)
- *COOLER PBA - HYD 2 (illuminated)
- *Nr - MONITOR
Hydrulic System Overtemperature
Indications:
- HYD 1 or 2 TEMP HI caution message and aural tone
- Hydraulic pressure normal
Procedures:
- *HYD 1/2 HT EXCH and HYD CONT circuit breakers - IN
If Circuit breakers are in and hydraulic temperature exceeds 115ºC:
- *Applicable hydraulic system failure - EXECUTE
Dual Hydraulic Failure
Indications:
- HYD 1 and 2 PRESS LO caution messages and aural tone
- HYD 1 and @ PRS guages low
- All SCAS and AFCS channels disengaged
- Increased control forces
Procedures:
- *Land Immediately
Hydraulic System Number 2 Failure
Indications:
- Grinding or howling noise from pump
- Fluctuating or low hydraulic system No.2 pressure.
- HYD 2 PRESS LO caution message and aural tone.
- HYD2 FLUID LVL LO caution message
- ROLL SCAS and AFCS disengaged
- MRGB TEMP HI and/or CBOX TEMP HI caution message(s) and aural tone if the COOLER PBA is in the HYD 2 position.
Procedures:
- *Establish airspeed less than 100 KIAS and angle of bank less than 35 degrees.
- *COOLER PBA - HYD 1 (blank)
Hydraulic System Number 1 Failure
Indications:
- Grinding or howling noise from pump
- Fluctuation or low hydraulic system No. 1 pressure.
- HYD 1 PRESS LO caution message and aural tone
- HYD 1 FLUID LVL LO cuation message
- PITCH and YAW SCAS and AFCS disengaged
- MRGB TEMP HI and/or CBOX TEMP HI caution message(s) and aural tone if the COOLER PBA is not in the HYD 2 position.
Procedures:
- *Establish airspeed less tha 100 KIAS and angle of bank less than 35 degrees
- *COOLER PBA - HYD 2 (illuminated)
Single-Engine Fire In Flight
Indications:
- Smoke
- Fumes
- Fire
- ENG 1 or 2 FIRE warning on flight page, ENG1 or 2 fire light on glareshield, and aural tone.
- ENGINE NUMBER 1 or 2 FIRE voice message
- Smoke trail observed during yaw maneuver.
Procedures:
- *Controls - ADJUST (to establish single-engine parameters).
- *Stores - JETTISON (as approriate)
- *Throttle (affected engine) - CLOSE
- *ENG fire PBA (affected engine) - ARM
- *READY/DISCH PBA - DISCHARGE. Illumination of the READY indicates fire bottle is ready for discharge.
Ensure indication changes to DISCH to show fire bottle discharged.
6.*FUEL PBA (affected engine) - CHECK CLOSED.
Single-Engine Failure In Flight
Indications:
- Left yaw
- Nr decreases
- ENG 1 or 2 OUT warning light
- NUMBER 1 ENGINE OUT or NUMBER 2 ENGINE OUT voice message
- Affected engine instruments decrease.
- RPM warning light and aural tone.
- ROTOR RPM voice message.
Procedures:
- *Controls - ADJUST
- *Stores - JETTISON (as appropriate)
Dual- Engine Failure in Flight
Indications:
- Left yaw
- Rapid Nr decay
- Rapid settling
- ENG 1 out ENG 2 OUT warning lights
- NUMBER 1 ENGINE OUT and NUMBER 2 ENGINE OUT voice messages
- Engine instruments for both engines decrease
- RPM warning light and aural tone
- ROTOR RPM voice message.
Procedures:
- *Autorotation - ACCOMPLISH
Engine Fire on Start (External)
Indications:
- ENG fire PBA (affected engine) illuminated and voice alert
- Smoke
- Fire
Procedures:
- *Throttles- CLOSE
- *START PBA’s - OFF (blank)
- *FUEL 1 and FUEL 2 PBA’s - CLOSE
- *ENG fire PBA (affected engine) - ARM
- *READY/DISCHARGE PBA- DISCHARGE. Illumination of the READY indicates fire bottle is ready for discharge. Ensure indication changes to DISCH to show fire bottle dishcarged.
Compressor Stalls
Indications:
- Affected Engine bands/pops
- No throttle response
- Decreasing or erratic Ng
- Increasing or erratic MGT
Procedures:
- *Controls - ADJUST (to maintain Nr, reduce power, and establish single-engine parameters)
- *Stores- JETTISON (as appropriate)
Engine Np Underspeed
Indications:
- Left Yaw
- Nr/Np droops if power demand exceeds single-engine limit
- Np mismached with Nr at 100 percent or slightly drooped.
- MGT decrease
- Ng decrease
- Torque split
Procedures:
- *Controls - ADJUST (to control Nr)
- *Stores - JETTISON (as appropriate)
Engine Ng/Np Overspeed
Indications:
- Right yaw
- Increase in Ng, Np, and Nr
- increase in MGT
- Torque Split
- ENG 1/2 Np or Ng OVERSPEED caution message and aural tone
- Possible rotor RPM warning light, ROTOR RPM HI warning message and aural tone.
- The good engine reduces power and Np of the overspeeding engine may increase well over 100 percent.
Procedures:
- *Controls - ADJUST (to control Nr)
Engine Hot Start
Indications:
- MGT reaches 867ºC during engine start
Procedures:
- *Throttle (affected engine) - CLOSE
- *FUEL PBA (affected engine) - CLOSE
- *START PBA (affected engine) - ENGAGE. motor until MGT decreases below 145ºC or 2 minute automatic timeout is reached
Single-Engine Failure During Takeoff
Indications:
- Left Yaw
- Nr decreases
- Engine indications (affected engine) decrease.
- ENG 1/2 OUT (affected engine) and RPM warning lights illuminated with voice messages
- ENG 1/2 OUT (affected engine) or RPM LOW warning legend is displayed on the FLT page warning banner
Procedures:
If Fly-away airspeed does not exist:
- *Controls - ADJUST (cushion aircraft for landing)
If fly-away airspeed does not exist:
- *Controls - ADJUST (to climb and establish/maintain autorotation parameters)
- *Single-engine failure in flight procedures - EXECUTE
Dual-Engine Fire in Flight
Indications:
- Smoke
- Fumes
- Fire
- ENG 1/2 FIRE warning on FLT page, ENG 1/2 fire light on glareshield, and aural tone.
- ENG 1/2 voice message
PROCEDURES:
- *Controls - ADJUST (to establish an autorotation)
- *Stores - JETTISON (as appropriate)
- *Throttles - CLOSE
- *FUEL 1 and FUEL 2 PBA’s - CLOSE
- *ENG fire PBA(s) - ARM
- *READY/DISCH PBA - DISCHARGE Illumination of READY indicates fire bottle is ready for discharge
Ensure indication changes to DISCH to show fire bottle discharged.
- *Autorotative landing - ACCOMPLISH
Electrical Fire
Indications:
- Somke or fumes
- Equipment failure
- Fire
- High AMPS indication
PROCEDURES:
- *Affected equipment - OFF and CBs pulled.
If fire persists:
- *INV 1/2 PBAs - OFF (illuminated)
If fire persists:
- *GEN 1/2 PBA’s - OFF (illuminated)
If fire persists:
- *Land immediately
Complete Electrical Failure
Indications:
- Loss of the non-essential electrical bus and associated mission computer and cockpit displays.
- Battery voltage drains.
- Loss of essential bus components to include the remaining mission computer and cockpit displays.
PROCEDURES:
- *Check BATT PBA - ON (blank)
if battery power is not restored:
- *Land immediately
APU Fire
Indications:
- APU fire PBA illuminated and voice alert.
- Smoke
- Fire
Procedures:
- *APU - OFF (blank)
- *APU fire PBA - ARM
- *READY/DISCH PBA - DISCHARGE. Illumination of READY indicates fire bottle is ready for discharge
Ensure indication changes to DISCH to show fire bottle discharged
- *Helicopter - SHUTDOWN
SCAS Hardover
Indications:
- Uncommanded excessive development of pitch, roll, and/ or yaw rates.
Procedures:
- *SCAS - DISENG
- *Aircraft Control - Regain
Emergency Shutdown
Procedures:
- *Throttles - CLOSE
- *FUEL PBAs - CLOSED (illuminated)
- *Rotor brake - ON
- *APU - OFF (blank)
- *Battery - OFF
Primary Oil Cooler Fail
Indications:
- PRI OIL COOLER FAIL caution message and aural tone
- Rise in CBOX and MRGB oil temperature
Procedures:
- *Cooler PBA - HYD 2 (illuminated)
