Why would customers need to enter manually enter charges?
Clinicians are not placing orders in an EMR, Clinicians perform services at outside place of service, interim clinical staff might not be trained on how to enter orders in an EMR, charges might be entered manually by analyzing a providers documentation to determine what services he performned

Errors must be fixed before going forward ; Warnings should be reviewed but do not need to be changed.
What is a claim?
