• Sx > 4 days/not improving
• systemically unwell (but not requiring admission)
• immunocompromise/high risk of complication due to conditions affecting: lungs, liver, heart, kidney or neuromuscular disease
• age < 2 y/o with BILATERAL otitis media
• otitis media w perforation and/or discharge in the canal

oral Amoxicillin for 5-7 days

If pen allergic: clarithro/erythromycin
