Endotracheal suctioning
Suctioning is the application of negative pressure in the airway through a collecting tube to remove secreation
Retained secreations or other semiliquid fluids are removed through suctioning
★To avoid complications rt needs to be
Pre oxygenate
Limit negative pressure
Use sterile technique
2 techniques
Open :sterile technique requires disconnecting the patient from the ventilator
Closed : technique uses a sterile in closed in line suction catheter attached to ventilator
Closed suction done is coma COVID ,tb
Deep suctioning: when the catheter is inserted untile resistance is met the. It’s withdrawn approximately 1 cm applying suction
Shallow suctioning:whenthe catheter is advanced to predetermined depth which is usualy the length of the airway
Indication
Need to maintain patency and integrity of artificial airway need to remove accumulated pulmonary secreations evidenced by the following
1.saw tool pattern-flow volum ventilator
Coarse crackles in trachea both are strong indicators for retained secreations
2 increased peak inspiratory pressure on volume control ventilation/decreased tidal volume or pressure control ventilation
3.deterioration of 02 saturation or blood gas value
4.visible secreations inthe airway
5 inability of pt to generate an effective cough
6.acute respiratory distress
7.aspiration. Of gastric upper airway secreations
8 when there is a need for sputum specimen to rule out desease conditions
Contraindications
Endotracheal suctioning is necessary procedure for patient with artifial airways so most of the contraindications are related to the risk of developing adverse conditions as a result of procedure
When indicated there is no absolute contra indications it may be only avoided if the patient is DNR or to avoid adverse reaction may be lethal
Complications
Decrease in the lung compliance and functional residual capacity
atelectasis
hypoxemia
hypoxia
Trauma to the tissue of trachea and bronchus
Bronchoconstriction
Bronchoscopasm
Increased icp
Hypertension hypotension
Cardiac dysarrythmias
