How to avoid perforation?
Pre-op planning

Always pre-curve initial small sized hand instruments
Do not skip instruments in sequence
Never rotate instruments in curved canals
Why does instrument separation occur?
Torsional stress
- extensive instrument surface encounters excessive friction within canal walls
Instrument tip larger than canal section being shaped
Cyclic fatigue
- tension and compression cycles on the instrument in curved canals leading to fatigue and failure
Give post-op complications of endodontic treatment

Don’t skip files
Don’t force files
Ensure file is passive before moving to bigger file
Recapitulate and patency file
Reservoir of irritant in pulp chamber while irrigating
How to avoid file fracture?
Know instrument limits and metallurgy
Use recognised technique
Pay attention to degrees of rotation of file
Lubricate canal
