Endo 2 Flashcards
hyperpituitarism
what is hyperpituitarism
symptoms associated with excess production of adenohypophysial hormone
usually due to:
1. pituitary tumours
2.ectopic tumour
how does pituitary tumour affect vision
it suppresses optic chiasma from below and cause bitemporal hemianopia (loss of vision from outer temporal visual fields)
excess SCTH cause?
cushing’s disease
excess TSH cause?
thyrotoxicosis
excess LH FSH cause?
precocious puberty in children
excess prolactin cause?
hyperprolactinaemia
excess GH cause?
gigantism (children)
acromegaly (adults)
Is prolactinoma a microadnemoa is it’s smaller than 10mm in diameter
yes
what is prolactinoma
a pituitary adenoma which cause excess secretion of prolactin, which suppresses GnRH sulsatility ==> no FSH LH
what receptor does lactotroph present
dopamine D2 receptor
give 2 examples of dopamine receptor agonists
bromocriptine
cabergoline
give 5 side effects of dopamine receptor agonists
nausea and vomiting
postural hypotension
dyskinesia ==> impairment of voluntary movement
depression
pathological gambling ==> impulse control disorder
how do you diagnose acromegaly
GH, IGF1 level test
paradoxical rise of GH after intake of oral glucose
what are the complications of acromegaly
obstructive sleep apnoea
hypertension
cardiomyopathy
increased risk of cancer
list drugs for acromegaly
somatostatin analogues: octreotide (endocrine cyanide)
dopamine agonists: cabergoline
