EN Definion
nutrition provided through GI tract via a cathater tube or stoma that delivers nutrients distal to the oral cavity
PN definion
provision of nutrients intravenously may be used with EN to meet requirements
En rational/criteria
If the gut works use it , for those who can’t or can’t eat enough must be considered first over PN
PN rational/criteria
reserved for nonfinctional or severly diminshed small bowel
Benefits of En over PN
preserved GI barrier and immune function
reduced catabolic response and cost
decreased rates of infection and LOS
better glucose control
CPN
catheter tip placement in large blood flow vein such as superior vena cava used in high osmolarity solutions
PPN
catheter tip placement in small vein typically in the hand or forearm
short term limited on liquids that can b used
concentrated solutions cant b admistered

no impact on motality
greater risk of GI complications ( D N V GERD bloating)

short term 3-4 weeks
normal GI function required
easy to insert

preferred for long term nonsurgical placement technique
Step 3 and 4 NG insertion
Measure insertion length
