EM No. 2 Flashcards
what does ASD do to S2?
fixed split S2
what is the splitting abnormality of S2 that involves splitting on expiration?
paradoxical splitting
what is the splitting abnormality associated with a split heard on both inspiration and expiration?
fixed splitting
i say right ventricular dysfunction you say what about s2 splitting abnormalities?
fixed splitting
i say pansystolic murmur you say? (2)
MR and TR
i say Mid-systolic ejection murmurs you say? (2)
AS and PS
I say late systolic murmur you say?
MVP
AS disappears over the sternum and then reappears at the apex (mimicking MR) - what is the phenomenon called?
Gallavardin
what does the handgrip maneuver do?
The handgrip maneuver increases afterload by squeezing the arterioles and increasing Total Peripheral Resistance. This is similar to the squatting maneuver (which also increases preload). The main difference is that the hand grip test does not increase venous return.
which two murmurs are typically increased with the handgrip maneuver?
AR and MR
hyponatremia and low urinary sodium equals ?
psychogenic polydipsia
deficiency is an inherited disorder that can cause lung disease in adults and liver disease in adults and children.
Alpha 1-antitrypsin deficiency
what is the most common cause of secondary HTN in the elderly?
renal artery stenosis
i say diastolic bruit in the epigastrum that radiates laterally - you say?
renal artery stenosis
how do you differentiate the systolic ejection crescendo decrescendo mumurs of HOCM and AS?
HOCM does nOT radiate to the neck
