Inhibition of platelet aggregation
TXA2 is a potent vasoconstrictor and platelet aggregator, must balance between the two
Latanoprost, Travaprost, Bimatoprost
PGF2a analogs
Resistant cases of open angle glaucoma
Itchy red painful eyes, changes eye color
Lipooxygenase pathway
Blocking cyclooxygenase will inc production of leukotrienes and block production of prostaglandins
Leukotrienes will cause bronchoconstriction (Asthmatics!!!)
COX 1 v 2 inhibition
Cox 1: antiaggregation
Cox 2: (inhibiting PGI2 formation) proaggregation, vasoconstriction
Aspirin
Irreversible acetylates cyclooxygenase
Anti inflammatory, antipyretic, analgesia, antiaggregation
Irreversibly inhibits cox in platelets (bronchospasm)
Salicylism: tinnitus and wide anion gap acidosis
Aspirin and NSAIDs
SE
Cox1: GI bleed, antiplatelet, thrombosis
Cox2: THROMBOSIS
Cox2 Selective NSAIDs are cardiotoxic
Acetaminophen
No resp or renal effects
Toxicity: chronic alcoholism induces CYP450 2E1 and can deplete glutathione stores (malnutrition)
N-Acetylcysteine is antidote for toxicity
