Doctor examines pt (physical exam) and asks more in-depth questions (PMHx, SHx, FMHx, etc.). HPI and ROS are important here
Differential Diagnosis
Doctor makes suggestions as to what the reasons for the symptoms are
Orders & Treatment
Doctor orders tests and decides treatments
Medical Decision Making MDM
Doctor makes a decision on the diagnosis
Disposition
Where patient goes after visit
HPI
History of Present Illness
During the examination, take concise notes. Start with “x y/o (gender) presents with (symptom)”
ONLY PUT HISTORY HERE
This comes first and determines the whole visit
ROS
Review of symptoms
Overviews positive and negative symptoms; lacks context, includes all mentioned symptoms in HPI
Should take 1 min
Physical exam
Pt gives subjective info; provider gives objective info
Doctor will INSPECT, PALPATE, and AUSCULTATE
Critical care
Ask provider how many minutes of critical care time were provided
Scribe attestation
