What are 50% of EBV cases?
asymptomatic
What has EBV been linked to?
neurologic sequelae, lymphoproliferative and neoplastic diseases
How is EBV spread?
oropharyngeal secretions, blood transfusions
What age has highest rate of EBV?
15-24 yo
What is the incubation for EBV?
4-6 wks
How does EBV present in young and infants?
mild pharyngitis
How does acute IM present?
posterior lymph nodes, fever, fatigue, myalgia, pharyngitis, splenomegaly and hepatomegaly, tonsillitis, morbilliform rash
What do you not give with EBV?
cillin abx - causes rash
How do you dx?
rapid mono - detects IgM - heterophile 90% pts
When do heterophile peak?
2-6 wks after infection
What will you see on CbC?
leukocytosis, lymphocytosis, atypical lymphocytes
What will you see on CMP?
elevated aminotransferases, elevated alkaline phosphatase, elevated bilirubin
What is the tx?
avoid abx, self limiting
When do you use prednisone?
tonsillar hypertrophy
What should EBV pt avoid?
strenuous activity for 4 wks - could rupture spleen
