E1 : Valvular Heart Disease & Pericarditis Flashcards
diastolic murmurs
- aortic regurgitation / insufficiency
2. mitral stenosis
systolic murmurs
- mitral regurgitation / insufficiency
- aortic stenosis
- asymmetric septal hypertrophy (ASH) / idiopathic hypertrophic sub aortic stenosis (IHSS)
etiology of mitral stenosis
rheumatic fever
symptoms associated w/mitral stenosis
- dyspnea
- orthopnea
- pulmonary edema (due to dilated LEFT ATRIUM)
when does complete incapacity occur w/mitral stenosis?
~ 7 years
how long is the onset of symptoms delayed w/mitral stenosis?
15-20 years
etiology of mitral regurgitation / insufficiency
- rheumatic fever
- papillary muscle dysfunction secondary to MI
- endocarditis
- mitral valve prolapse
- Redux (diet drug that causes pure right heart failure)
acute mitral regurgitation =
pulmonary edema
chronic mitral regurgitation =
slow onset symptom of CHF
etiology of aortic stenosis
- rheumatic fever
- congenital bicuspid valve
triad of symptoms associated w/aortic stenosis
- syncope
- angina
- left ventricular failure
SURVIVAL < 5 YEARS
drugs used to treat aortic stenosis
- digoxin
- diuretics
- no salt
indications for surgery to treat aortic stenosis
- active and still have excellent QOL
- gradient across valve > 50 mm
etiologies of aortic regurgitation / insufficiency
- rheumatic fever
- syphilitic aortitis
- ankylosing spondylitis
- marfans
symptoms of aortic regurgitation / insufficiency
volume overload (LV failure)
CXR indications for surgical treatment of aortic regurgitation / insufficiency
- MOD LV failure
- LV hypertrophy w/strain on EKG = SVI + RV5 > 35 mm
- pulse pressure > 100
etiology of ASH / IHSS
genetic
symptoms of ASH / IHSS
- syncope
- angina
- difficult on exertion
- sudden death
drugs used to treat ASH / IHSS
- beta blocker
- CCB
- (avoid digoxin)
symptoms of pericarditis
- sharp chest pain
- rub
- diffuse EKG changes (ST concavity)
etiologies of acute pericarditis
- idiopathic
- trauma (blunt to chest)
- CVD (rheumatic fever, SLE)
- infection (bacterial, TB, fungal, VIRAL)
- cancer w/metastases (lung, breast, bad prognosis)
- radiation through chest
- uremia
- post thoracotomy
etiologies of chronic pericarditis
- idiopathic
- infection
- CVD
- tumor/metastases
- radiation
- uremia
