E1 Bronchiectasis, CF, Interstitial Lung Disease Flashcards
most common cause of bronchiectasis
cystic fibrosis
most accurate test for cystic fibrosis
sweat chloride test (> 60 meq/L on at least 2 tests)
treatment for allergic bronchopulmonary aspergillosis
- oral prednisone
- itraconazole
best initial test for bronchiectasis
CXR (tram tracks)
most accurate test for bronchiectasis
chest CT
treatment for bronchiectasis
- chest physiotherapy
- abx. - treat infections as they arise
- surgical resection of focal lesions
- inhaled bronchodilators (most often albuterol)
pneumoconioses type associated w/sandblasting, rock mining, tunneling
silicosis
pneumoconioses type associated w/shipyard workers, pipe fitting, insulators
asbestosis
pneumoconioses type associated w/cotton
byssinosis
pneumoconioses type associated w/electronic manufacture, nuclear reactors, defense and aerospace industries
berylliosis
pneumoconioses type associated w/moldy sugar cane
bagassosis
syndrome associated with rheumatoid arthritis + pneumoconiosis
caplan syndrome
pneumoconiosis that is most likely to respond to prednisone
berylliosis
(3) types of bronchiectasis
- cylindrical
- varicose
- cystic
sputum cultures of cystic fibrosis
- nontypable h. influenza
- pseudomonas aeruginosa
- staph aureus
- burkholderia cephacia
treatment for cystic fibrosis
- antibiotics (even inhaled ahminoglycosides)
- albuterol (inhaled)
- inhaled recombinant human deoxyribonuclease (rhDNase)
- pneumococcal and influenza vaccines
- rigid bronchoscopy and embolization for gross hemoptysis
allergic bronchopulmonary aspergillosis are mainly patients with :
- atopic eczema
- asthma
- recurrent episodes of brown-flecked sputum
- transient infiltrates on CXR
diagnosis for bronchpulmonary aspergillosis
- aspergillus antigen skin test
- aspergillus antibody blood test
- peripheral eosinophilia
- elevated serum IgE
- CXR or CT pulmonary infiltrates
