Ix for oropharyngeal dysphagia?
-Video swallow
>provides function information
>can identify pharyngeal pouch
Ix for Oesophageal dyshagia?
- Gastroscopy
- Barium Swallow
- CT scan
- Oesophageal manometry
- Endoscopic U/S
Advantages of gastroscopy for investigation of dysphagia?
- Identify structural abnormality
- May be therapeutic (e.g. dilate stricture / remove foreign body)
When is barium swallow useful in dysphagia work up?
- Achalasia
- If pharyngeal pouch suspected
When is CT scan useful in dysphagia work up?
- -Assessment large hiatus hernia
- Extrinsic compression of oesophagus
When is manometry useful in dysphagia work up?
-Assessment of achalasia or DES
When is endoscopic u/s useful in dysphagia work up?
-Characterise lesions in the wall e.g. Gastrointestinal Stromal tumours (GIST)
What is the cause of a pharyngeal pouch?
-Dysfunction / spasm of upper oesophageal sphincter
Hx features suggestive of pharyngeal pouch?
- Gurgling in neck
- Brings up previously eaten foods
Treatment of pharyngeal pouch?
Cricopharyngeal myotomy via
i) open procedure in the neck
ii) endoscopic transoral myotomy
