What is angiodysplasia?
A
Degenerative vascular malformation of the GIT characterised by fragile and leaky blood vessels resulting in GI bleeding and anaemia.

What are the RFx for angiodysplasia?
A
- Age
- ESRD on dialysis
- von-Willebrand’s disease
- Aortic stenosis
- Scleroderma

Where is angiodysplasia most commonly found in the colon?
A
Mostly proximal colon; also jejunum and transverse colon.

Ix in angiodysplasia?
A
- FBE: anaemia, low Hb
- Blood type and cross match
- UEC (high urea/Cr)
- OGD/colonoscopy: abnormal epithelium with small lesions with irregular edges and draining vein.
- Faecal occult blood: +ve
- Radionuclide scans using technetium Tc-99m

DDx angiodysplasia?
A
- Diverticular disease
- CRC
- Haemorrhoids
- CD/UC

Q
How should haemodynamically unstable pts be managed?
A
-Interventional endoscopy or
-Angiogrpahy with embolisation or
-Colonoscopy +/- right hemicolectomy or
ALL with supportive care: IV access + fluids/transfusion; blood typing and cross match.

What are the complications of angiodysplasia?
A
- Bowel perforation (interventional)
- Re-bleeding
- Ischaemic bowel disease
