What is angiodysplasia?

A

Degenerative vascular malformation of the GIT characterised by fragile and leaky blood vessels resulting in GI bleeding and anaemia.

What are the RFx for angiodysplasia?

A
  • Age
  • ESRD on dialysis
  • von-Willebrand’s disease
  • Aortic stenosis
  • Scleroderma

Where is angiodysplasia most commonly found in the colon?

A

Mostly proximal colon; also jejunum and transverse colon.

Ix in angiodysplasia?

A
  • FBE: anaemia, low Hb
  • Blood type and cross match
  • UEC (high urea/Cr)
  • OGD/colonoscopy: abnormal epithelium with small lesions with irregular edges and draining vein.
  • Faecal occult blood: +ve
  • Radionuclide scans using technetium Tc-99m

  • Diverticular disease
  • CRC
  • Haemorrhoids
  • CD/UC

How should haemodynamically unstable pts be managed?

A

-Interventional endoscopy or
-Angiogrpahy with embolisation or
-Colonoscopy +/- right hemicolectomy or
ALL with supportive care: IV access + fluids/transfusion; blood typing and cross match.

What are the complications of angiodysplasia?

A
  • Bowel perforation (interventional)
  • Re-bleeding
  • Ischaemic bowel disease
