Onset of primary dysmenorrhea
Usually within 3 to 6 months
Cause of primary dysmenorrhea
Excess prostaglandin E2 secretion leading to painful uterine contractions
Cause of secondary dysmenorrhea
Usually disease of uterus or pelvis (endometriosis, adenomyosis, uterine fibroids, PID, use of IUDs)
Onset of secondary dysmenorrhea
Affects wmn older than 25
Prevalence of dysmenorrhea
Affects more than half of women of reproductive agen
Treatment of primary dysmenorrhea
1- NSAIDs before expected menses
2- oral contraceptives
3- applying heat
4- if resistant to above, tocolytic agents, CCBs, or progestogens
Treatment of secondary dysmenorrhea
Tx underlying condition, hysteroscopy, dilation and curettage
