Secondary amenorrhea
Absence of menses for 6 months or longer
Primary amenorrhea
Absence of spontaneous menstruation by age 16
Mc cause of secondary amenorrhea
Pregnancy
4 types of primary amenorrhea
With no secondary sexual character, with breast development but no hair, with normal secondary sexual characteristics, with incomplete secondary sexual characteristics
Amenorrhea with no secondary sexual characteristics
Suggests gonadal a genesis or dysgenesis, ovarian resistance syndrome, galactosemia, gonadotropin-releasing hormone deficiency, Cns mass, lesion, hyperprolactinanemia
Amenorrhea with breast development but hair
Suggests androgen insensitivity
Amenorrhea with normal secondary sexual characteristics
Suggests imperforate hymen, transverse vaginal septum, cervical or mullerian agenesis
Amenorrhea with incomplete secondary sexual characteristics
Tumor of hypothalamus, pituitary, hypothyroidism, premature ovarian failure, hyperprolactinemia
MC cause of secondary amenorrhea
Pregnancy
Other causes of secondary amenorrhea
Drug use, stress, significant weight change, excessive exercise
Causes of secondary amenorrhea with normal estrogen level
PCOS, Asherman’s syndrome
Lab studies of amenorrhea
Pg test, FSH, estrogen, prolactin, testestrone levels, progesterone challenge test (to determine the presence or absence of estrogen), thyroid, MRI, CT
