Unilateral calf swelling Unilateral calf tenderness Mild fever Pitting oedema Prominent superficial veins
How do you find out if a calf is swollen?
Measure both calves 10cm below tibial tuberosity
> 3cm difference = swollen
What are differentials for DVT?
Muscle strain
Lymphangitis
Cellulitis
What is phlegmasia cerulea dolens?
Painful blue leg swelling
What is a provoked DVT?
Within 3 months of a major clinical risk factor
What is an unprovoked DVT?
Occurs without prior clinical trigger
Investigations for suspected DVT
Bloods
- FBC
- LFTs
- U+Es
- INR
- aPTT
Doppler venous flow
What is the Wells score?
Score for risk of DVT
<2 = low probability >/=2 = moderate probability >/=3 = high probability
What are the criteria for the Wells score?
Cancer Bedridden Calf swelling Pitting oedema Previous DVT
What test can you do if its a low probability?
D-dimer
Is D-dimer a good test?
Sensitive but not specific
If low - proves no DVT
If high - requires further investigations
What can cause a high D-dimer?
MI
DIC
Stroke
Pregnancy
What are potential treatments for a DVT?
DOAC
LMWH
