What are the symptoms of ACS?
Central/left sided chest pain Heavy/tight pain Dysponea Sweating Tachycardia N+V
What is the immediate management of ACS?
MONA
Morphine
Oxygen
Nitrates
Aspirin - 300mg
What investigations do you do for ACS?
Bloods
- FBC
- U+Es
- troponin
- CK
ECG
CXR
When does troponin peak?
6 hours from pain
How do you manage a STEMI?
Aspirin + clopidogrel
LMWH
PCI - ideally within 90 mins
If PCI not available - tPA
- tenecteplase
- alteplase
- streptokinase
How do you manage an NSTEMI?
300mg aspirin
GP IIb/IIIa receptor antagonist - clopidogrel
Coronary angiography
What is secondary prevention of ACS?
Aspirin Clopidogrel B-blocker ACEi Statin
What is Dressler’s syndrome?
Autoimmune condition that occurs 2 weeks after MI
What are the symptoms of Dressler’s syndrome?
Sharp chest pain - relieved by leaning forward
Fever
Raised ESR
How is Dressler’s syndrome managed?
NSAIDs
Cultrasine
