Epidemiology of DVT
1 in 1000 annually
Age 70+. M=F. Females have a higher recurrence rate.
Risk factors cancer >60 years smoking surgery trauma thrombophilia, pregnancy, recent, family or personal Hx, Oral Contraceptives HRT
Aetiology of DVT
venous occulsion of deep vein in periphery
impaired venous return
Pathogensis
venous occulsion due to thrombus -
thrombus can be RBC,fibrinogen, platelets
manefestations
Mostly in lower leg but can manifest elsewhere in body.
Unilateral usually. (difference of 3cm between increased chance).
ALSO
Swelling of entire leg
Tenderness along vein distribution in Calf or thigh = increased probability
Symptoms
tenderness along veins, calf pain.
Signs
dilated superficial collaterals, swelling of entire leg, pitting oedema. +’ve h0mans sign.
there may be low grade fever.
Complications
Pulmonary embolism, chronic venous insufficiency, post phlebitic syndrome
Prognosis
3% risk of fatal Pulmonary Emob. Good prognosis if caught early.
