Drugs for Regulating Serum Lipid Levels Flashcards
Why do we want to reduce serum lipid levels
To reduce incidence of Atheroscelrois, to icrease insulin sensitivty and other hormones
What drug decreases de novo cholesterol synthesis
The Statins, which inhibit HMG CoA reductase
- Lovastatin
- Atorva
- Simva
- Fluva
- Pitava
- Rosuva
- Prava
Problems with adherence with statins
Low perceived effect
Other Drugs
Bile acid sequestrants, Ezetimbe, Nicotinic acid/Niacin r Vitamin B3, Fibrates and Fish Oil
What is the function o bile acid sequestrants?
Bind bile in the gut to prevent fat/cholesterol absorption and include cholestyramine nd colestipol
What is the function of Ezetimibe
To specifically inhibit cholesterol absorption
What is the function of Nicotinic Acd, Niacin and it B3
Decreases secretion of VLDL fom liver leadig to lower LDL in plasma, will also increase HDL
What is the function of Fibrates
Include gemfibrozl and fenofibrate and they ct on uclear rceptors to act o PPARa rceptor to increase synthesis of lipoprotein lipase to decrease LDL
What is the function of Fish Oil
Contains Omega 3 fats and decreases TGAs and VLDL
mechanisms of reducing LDL
Increasing the receptor expression on cells or inhibiting de novo synthesis or decreasing absorption
Targets that need to be obtained from treatment
Treat someone who is over 5.5mmol/L and aim to get below 4mmol/L.
