Morphine sulfate (adult dose)
IV/IO/IM-1.0 to 4.0 mg increments over 1 to 5 minutes
Verapamil (adult dose)
IV/IO- 2.5 mg over 2 minutes, may repeat dose 5.0 to 10 mg in 15 to 30 minutes. MAX DOSE-30 mg
oxytocin (adult dose)
Infusion-10 to 20 U in 1,000 mL NS or LR
IM- 10 U
calcium gluconate 2.5% topical gel (adult dose)
If possible, patient should wash affected area and massage the gel into the skin

Torsades infusion- 1.0 to 2.0 gm in 50 to 100 mL over 5 minutes, may repeat dose 1.0 to 2.0 gm over 1 hour
Pre-term labor/pre-eclampsia/eclampsia infusion- 3.0 to 6.0 gm in 100 mL over 10 to 20 minutes, may repeat initial dose
Maintenance infusion- 1.0 to 4.0 gm/hour
Reactive airway conditions- 2.0 gm in 50 mL over 5 to 10 minutes
Cardiac arrest IV/IO- 1.0 to 2.0 gm over 1 to 2 minutes

Therapeutic dose range and responses
2.0 to 5.0 mcg/kg/minute- Negligible beta-adrenergic response
5.0 to 10 mcg/kg/minute- Beta-adrenergic responses- positive chronotropic effects, positive inotropic effects
10 to 20 mcg/kg/minute- Alpha-adrenergic responses- vasoconstriction, increased blood pressure and preload
>20 mcg/kg/minute- may increase heart rate and myocardial demand to undesirable limits
Etomidate (adult dose)
IV/IO- 0.2 to 0.6 mg/kg (usually 20 mg) over 30 to 60 seconds
Furosemide (adult dose)
If patient is prescribed furosemide initial IV/IO dose is 2 times the patients total daily dose (usually 40 to 80 mg)
Not prescribed IV/IO- 0.5 to 1.0 mg/kg (usually 30 to 40 mg)
Ketamine (adult dose)
IV/IO- 1.0 to 2.0 mg/kg over 1 minute
IM- 2.0 to 4.0 mg/kg
Lidocaine (adult dose)
V-fib and pulseless V-tach IV/IO- 1.0 to 1.5 mg/kg
May repeat dose of 0.5 to 0.75 mg/kg every 3 to 5 minutes during resuscitation MAX DOSE- 3.0 mg/kg
Maintenance infusion- 2.0 to 4.0 mg/minute
ET- 2.0 to 2.5 time the bolus dosage diluted in 10 mL normal saline

May repeat dosage
Ondansetron (adult dose)
Oral/IV/IO- 4.0 to 8.0 mg
Adenosine (adult dose)
IV/IO- 6.0 mg rapid bonus over 1 to 3 seconds followed by 20 mL normal saline flush
If no respond after 1 to 2 minutes administer- 12 mg rapid bonus over 1 to 3 seconds followed by 20 mL normal saline flush

Cardiac arrest IV/IO- 300 mg, if no response administer 150 mg over 3 to 5 mintes
Tachydysrhythmias infusion- 150 mg in 50 mL D5W infused over 10 minutes

IV/IO/IM- 0.5 to 1.0 mg

Bucal, sublingual-15 to 50 gm, may repeat dose
