Drug class of Dopamine
Sympathomimetic, vasopressor, inotropic agent
Dopamine’s mechanism of action
Immediate metabolic precursor to nor-epinephrine. Produces positive inotropic, and chronotropic effects. Dilates renal and splanchnic vasculature. Constricts systemic vasculature, increasing blood pressure and preload. Increases myocardial contractility and stroke volume.
Indications of Dopamine
Cardiogenic and septic shock, hypotension with low cardiac output states, distributive shock, symptomatic bradycardia
Contraindication of Dopamine
Hypovolemic shock, pheochromocytoma (adrenal tumor that causes persistent or episodic high blood pressure), tachydyrhythmias, ventricular fibrillation.
Adverse effects of Dopamine
Extravasation may cause tissue necrosis. Headache, anxiety, dyspnea, dysrthythmias, hypotension, hypertension, palpitations, chest pain, increased myocardial oxygen demand, PVCs, nausea, and vomiting.
Precautions of Dopamine
Incompatible with alkaline solutions, MAOI’s &
Bretylium will enhance effects, beta blockers
Will antagonize effects. Do not give with
Phenytoin (hypotension, bradycardia, seizures)
Adult dose and admin of Dopamine
2 to 20 mcg/kg/min titrate to effect
2 to 5 mcg/kg/min (low dose)
5 to 15 mcg/kg/min (mid dose)
15 to 20 mcg/kg/min (high dose)
Pedi dose of Dopamine
2-20 mcg/kg/min IV/IO slowly titrated to effect
