DOMS
delayed onset muscle soreness
what is DOMS
soreness and discomfort 24-72 hours after an unaccustomed bout of exercise or activity
when is DOMS more likely to occur
repeated eccentric efforts
potential mechanisms are
unknown
mechanical, chemical and/or neurological
what does early research blame DOMS on
lactic acid buildup after exercise
but this has been largely disproven
after 1 hour of recovery
nearly all lactic acid is removed from skeletal muscle and blood
what does current research suggest
DOMS is linked to some form of contract induced, mechanical disruption of muscle fibers and CT
mechanical disruption of muscle fibers and CT –> current research
elevate serum creatine kinase present for several days after exercise
accompanied by inflammation and local edema
there is a temp loss of
strength
both precedes and persists after sensation of soreness
what is marginally successful at best
prevention and tx
best strategies
frequently repeating and progressing exercise intensity
repeated bout effect
bout of eccentric exercise may protect the muscles from subsequent bouts of exercise
tx for DOMS
continuing exercise
modalities
nutritional supplements
compression sleeves and salicylate topicals
continuing exercise –> tx
that caused the DOMS
does not worsen damage or slow recovery process
modalities –> tx
heat, ice, stim, massage
all show mixed evidence at best
nutritional supplements
limited effectiveness
compression sleeves and salicylate topicals
may reduce severity and hasten recovery
what should we educate the pt on
this is normal and an expected response to exercise
what should we ensure
that sxs are DOMS and not reinjury or rhabdomyolysis
DOMS is a
general sensation of achiness
should not replicate sxs of injury
what is rhabdomyolysis
emergent condition
more achiness that lasts for several dats
pt feels sick
pt has dark color urine
refer out immediately
DOMS is typically a sign that
exercise dosage is effective
practical advice on DOMS
adjust as necessary to pt tolerance
exercise w/ DOMS is safe
