What is diverticular disease?
It is the symptomatic presence of diverticula.
What is a diverticula?
An outpouching of the bowel wall.
Where is a diverticula most likely to occur?
Where the blood vessels enter the wall. This is because this is a weak point in the wall.
What is diverticulitis?
The inflammation of the diverticula.
How do diverticula form?
Straining of the colon forces the mucosa to herniate through the muscle layers at weak points. It is thought to be due to the lack of dietary fibre in the western world
How does diverticulitis occur?
Bits of stool can become lodged in a diverticula and this becomes inflamed
Where do most diverticula form?
In the descending and sigmoid colon (95%)
How many people have diverticulosis in the western world?
30% of 60 year olds
How is diverticular disease diagnosed?
Often incidentally on colonoscopy.
How do we confirm diverticulitis?
CT scan of abdomen
What do you not do to investigate diverticulitis?
Colonoscopy
What investigations do we do in diverticulitis?
Abdo exam PR exam FBC - raised WCC, CRP Full obs - temp Erect CXR - free air AXR - free air CT scan
What are the symptoms of diverticular disease?
Altered bowel habit
Left sided pain relieve by defecating
What are the symptoms of diverticulitis?
Altered bowel habit Left sided pain relieved by defecating Fever Rectal bleeding Generalised peritonism
What can we do for diverticular disease?
Not much really.
High fibre diet
Anti-spasmodics e.g Mebeverine 135mg TD PO
