A

A

Where the blood vessels enter the wall. This is because this is a weak point in the wall.

A

The inflammation of the diverticula.

How do diverticula form?

A

Straining of the colon forces the mucosa to herniate through the muscle layers at weak points. It is thought to be due to the lack of dietary fibre in the western world

A

Bits of stool can become lodged in a diverticula and this becomes inflamed

Coamoxiclav

Ciprafloxacin and metronidazole if penicillin allergy
Fluids only for 2-3 days
Analgesia

A

Abscess

How would a diverticular abscess present?

A

Swinging fever
Leucocytosis
Localising signs

What must you remember if you suspect an abscess but can seem to find one?

A

Pus somewhere, pus nowhere = PUS UNDER THE DIAPHRAGM

This is a subphrenic abscess and you need to do an urgent ultrasound

When is surgery needed?

A

Generalised purulent peritonitis
Generalised faecal peritonitis

Sometimes:
Pelvic abscess or walled off diverticula

What are some indications for surgery in diverticulitis?

A

Stenosis
Fistula
Recurrent bleeding
