Disorders de la Ventilation Flashcards
Primary Alveolar Hypoventilation
Ondine's Dz Rare Inadequate alveolar ventilation Worse during sleep Normal systems Congenital or trauma based
Primary Alveolar Hypotension Patient profile
Usually non-obese
Males from 30-40
Lethargy, HA, Somnolence
Evidence of pulmonary HTN
Primary Alveolar Hypotension Tx
O2
Positive pressure ventilation
Diamox, theophylline (resp stimulants)
Diaphragm pacing
Pickwickian Syndrome
Blunted ventilatory drive and increased mechanical load from obesity.
Voluntary hyperventilation maintains normal range
Most likely also have sleep apnea
Pickwickian Tx
Weight loss
CPAP, Bipap
Respiratory stimulants
Obstructive sleep apnea (OSA)
Most common sleep-related breathing disorder
20-30% of males
10-15% of females
More common in young african americans
OSA Patho
Recurrent collapse of airway during sleep
Cessation of airflow
Poor sleep quality
OSA risk factors
Obesity Old age Smoking Pregnancy Renal, lung, heart dz
OSA treatment
Weight loss
Sleep positioning
CPAP** (TOC)
Bipap
Central Sleep Apnea
Sleep apnea where brain forgets to send impulses for the body to breathe.
Common post-stroke, TBI, CHF
Hypopnea
Decreased airflow with a drop in O2 sat of at least 4%.
Apnea
Breath cessation for at least 10 seconds with decreased O2 sat
