Causas de riesgo vital comunes en disnea
- Trauma
- SNC: Intoxicación
- Vent: OVACE, anafilaxia, Angioedema, Asma, EPOC ex, Neumonia, NxTx, TEP
- Corazon: ICAD, SCA, Arritmia, Taponamiento
Signos clínicos de paro respiratorio inminente
- Compromiso de Conciencia
- Inhabilidad de mantener el esfuerzo respiratorio (sentados-tripode, taquipnea, tiraje, diaforesis)
- Cianosis
¿Qué sugiere una caida abrupta de la SpO2 asociada a ejercico en inmunosuprimido?
Neumonia por PCP
¿Como se determina el pulso paradojico?
Disminución de la PAS > 10 mmHg entre Espiracion a Inspiracion profunda.

¿Por que se produce el pulso paradojico?
Disminución del volumen sistólico ventricular izquierdo por desviacion septal hacia el mismo durante inspiracion profunda y por ende mayor llenado VD en cavidad pericardica restringida
BNP: valores limites
< 100: descarta ICAD (VPN 90%)
> 500: muy sugerente de ICAD
¿Se debe limitar el aporte de O2 en EPOC exacerbado para evitar hipocapnia?
No
EtCO2 (Capnigrafia telespiratoria): utilidad en disnea
Monitorizar mejoria (disminucion) o empeoramiento (aumento) de la funcion ventilatoria
Enfrentamiento disnea
- A: dificultad de VA
- B: O2, ¿necesita soporte ventilatorio?, STOPMAID
- C: monitores y VVP
- Determinar causas rápidamente tratables: NxTx a tensión, taponamiento, OVACE)
¿Sirve usar VMNI en ICAD y EPOC?
Sí, pero solo cuando responden rapidamente a terapia medica inicial, de lo contrario se debe usar VMI
