What is discharge planning?

A
  • Planning that takes place for a patient to return to their own home (or alternative location) following medical clearance.
  • Process of preparing the client/patient and getting everything organised ready to leave the hospital.

What is involved in discharge planning? How do you get it organised and plan for it?

A
  • Complete initial Ax to identify OP issues, abilities and limitations, clients life situation (environmental/cultural etc). determine client preferences and set goals
  • Once you have the information you can:
  • Identify current functional status and contrasts with previous level of function
  • Identify what client needs to be able to safely ‘do’ (prioritise)
  • Identify therapy interventions that will bridge the gap
  • Identify available assistance/equipment once home
  • Assist to arrange any equipment and services required
  • Refer on to other health professionals as required

Where do you get relevant information from?

A
  • Read clinical notes/referral
  • Discussion with other hospital staff
  • Informal discussion with client, family/carer
  • Formal and informal Ax: initial Ax, functional Ax, environmental Ax/HV etc

A

Initial OT assessment or stage 1 of OPPM

  • In rehab or post acute setting: on admission
  • In acute setting: after person is medically stable

What does a home assessment involve?

A
  • Discussion with client and/or family in relation to home environment. Assessment of the client in the home environment if possible.
  • Assessment of internal and external environment. eg. access, hazards, ability to complete tasks, transfers, mobility, safety, equipment and home modifications required
