- Cannot be prevent by vaccines, cured by medication
- Health damaging behaviours contribute to chronic diseases

What is self-management?
What person with chronic condition does to manage their condition and the actions they take to cope with impacts of their condition
What is self-management support?
- What others such as services, health professionals, family, friends, carers do to support person to self manage
- Do this by providing physical, social, emotional support
What are the 6 principles of self management?
1) Know your conditions and its treatments
2) Have active involvement in decision making with GP or health professional
3) Follow care plan that agreed upon with health professionals
4) Monitor symptoms associated with condition(s) and Respond to and cope with symptoms
5) Manage physical, emotional and social impact of condition(s) on your life
6) Live a healthy lifestyle
