What is the therapuetic and Pharmacologic classifications of Digoxin?
Therapeutic: antiarrhythmics, inotropic(force of contraction)
Pharmacologic: digitalis, glycosides
Dosage/route/frequency of Digoxin?
Adult:Dgitizing dose
IV:0.5-1mg
PO:0.75-1.5mg
Doses are given as 50% of initial dose in each of 2 subsequent doses at 6-12hrs
What are the indications of Digoxin?
Given for heart failure, a-fib, and a-flutter. Also paroxysmal atrial tachycardia
How does digoxin work?
It increase the force of the myocardial contraction. Prolongs refractory period of AV node. decrease the conduction of SA and AV nodes
What side effects are most likely for Digoxin?
CNS: Fatigue
CV: Arrhythmias, bradycardia
GI: Anorexi, n/v
What is priority nursing intervention to take prior to giving Digoxin?
It’s Digoxin take a Apical pulse for 1 minute(not 30seconds). Withhold the dose if rate is <60 and moitor ECG changes at east 6hrs after initial dose
What does the nurse need to teach the pt about Digoxin?
name, dose, route, purpose, teach pt and family how to take pulse and to notify MD if 100bpm
-take missed doses within 12hrs DO NOT double dose. If dose is missed outside of 12hr window, omit. Consult MD if doses for 2 consecutive days is missed
What other medications are a concern for pts taking digoxin?
Any meds that cause hypoK like loop diuretics and thiazde may increase risk of toxicity
