definition

A
  • a pathological condition occurring secondary to an underlying disease and characterized by inappropriate activation of the coagulation and fibrinolytic activation of the coagulation and fibrinolytic systems
  • there is consumption of fibrin, clotting factors ( esp factor V, VIII, prothrombin ) and platlets, resulting in continued bleeding + futher depletion of factors

etiology

A
  1. intrauterine fetal death
    - IU fetal death and retention of dead fetus for > 3 weeks 🡪 leakage of thrombop??? + thromboplastin-like substances into maternal circulation from degenerating placenta + as well as plasminogen activates from fetus + placenta
  2. placental abruption – in this case:
    i) hemorrhage + retroplacental clot formation result in depletion of fibrinogen, platlets, coagulation factors
    ii) damaged placenta + uterus release tissue thromboplastine + plasminogen activator into general circulation
  3. AF embolism – complication of artificial rupture of membrane, CS, external vergoon, causes
    i) at release of tissue thrombopastine
    ii) increase coagulation due to pro-coaggulant activates of AF
  4. PE, eclampsia – chronic co-aggulation abN 🡪 thrombocytopenia + increase fibrin degradation products ( due to platlet adherence to damaged endothelium )
  5. saline / septic abortion
    - release of tissue thromboplastin from placenta / necrotic tissue
    - septic abortion 🡪 also release of bacterial endotoxin ( phospholipids ) which is a pro-coaggulant
    i) Hypercoagulation – consumption of platlets and coagulant factor
    ii) Hypocoagulation – start bleeing

1. co-aggulation stage – stage during which there is release of thromboplastin + activation of clotting shunt formation
Subcompensation
2. fibrinogen depletion – during this stage, there is fibrinogen depletion even if thromboplastin are not present due to exhaustion of clotting system
- fibrinolysis – secondary to activation of fibrinolysis system by product of fibrin breakdownn 71
