What to look for on inspection
GAIT
Look at shoes - uneven wear or foreign objects
Feet - Hairlessness, pallor, decreased sweating, redness, skin ulcers, infection
look at toenails
check temp and CRT
Palpation for diabetic foot
Palpate bones and joints of the feet and ankles and watch patients face
Pulses for diabetic foot
palpate femoral, popliteal, posterior tibial, dorsalis pedis
Reflex
ankle jerk
Sensation for diabetic foot
Light touch eyes closed and then pressure with a monofilament on pulp of hallux, and then 1,3,5 metatarsal
Pin prick and temp describe only
Proprioception
feel temperature
vibration with tuning fork
Present diabetic foot exam
Today i completed a diabetic foot exam on this patient. On general inspection they were comfortable at rest. on palpation there was no tenderness, sensation was intact in both feet, to conclude, this is consistent with a normal diabetic foot exam, to complete I would examine the patients lower limbs neurologically and vascular and woudl carry out a fasting/random blood glucose and a Hba1c blood test
