What is diabetic foot syndrome?
Combination of microvascular disease, macrovascular disease, neuropathy and infection.
Macrovascular disease is predominantly below knee cf non-DM disease
What is the aetiology of diabetic ulcers?
Neuropathic: 45-60%
Ischaemic: 10%
Mixed neuroischaemic: 25-45%
Why are the pulses preserved in diabetic vascular disease?
Due to calcification in the vessel walls (media sclerosis) –> preserved pulses til late disease. Similar to effect of chronic renal failure.
Results in abnormally high ABPI- use toe pressure (less than 30mmHg)
What additional problems do diabetics undergoing angiography have?
Degree of renal impairment- worsened by contrast agents
Metformin- must stop prior to procedure due to risk of lactic acidosis
