Diabetes Pharm Flashcards
Name the four types of Insulin Preparations
Rapid-acting
Short-acting
Intermediate-acting
Long-acting
Name two types of Short-acting insulin preps
S.C. (Soluble crystalline zinc formulation. Starts in 30 min and peaks at 2-3 hours.
I.V. Prep
Describe the presented Intermediate-acting insulin
NPH (Neutral Protamine Hagedorn, or Isophane) Insulin
Onset in 2 hours and lasts 4-12 hours.
Unpredictable action….Not used clinically as much
Human Insulin Analogs (Rapid-acting)
Insulin LisPro
Insulin AspArt
Insulin Glulisine
Onset in 5-15 minutes. Peaks at 1 hour.
Basal Insulin Analogues (Long-Acting)
Glargine has a “peakless” action
Detemir has the most reproducible effect of the intermediate and long acting insulins.
Degludec is ULTRA long acting
Insulin delivery options
Injections
Pump
Inhaled
Metformin
Targets the Liver, Decreasing hepatic glucose output and lower fasting blood glucose levels.
Insulin Seceratogues
Targets the Pancreatic B Cells. Potentiating insulin secretion
Newer ones are the meglitinides
Thiazolidinediones (Glitazones)
Targets Adipose, muscle and liver insuling insulin sensitivity.
Alpha glycosides inhibitor
Target intestinal alpha glucosidases, reducing glucose absorption
GLP-1 based therapies
Receptor agonists- Potentiates insulin secretion, suppressed glucagon secretions, and impairs gastric motility.
DPP-4 Inhibitor- increases Incretins GLP-1 and GIP.
Amylin Agonists
Slow gastric Emptying, inhibit glucagon production
Kidney SGLT2
REducing glucose re-absorption by the kidney
Bile Acid binding Resins
Colesevelam
Bromocriptine (CYCLOSET)
Dopamine receptor agonist
