What is developmental dysplasia of the hip?
DDH is abnormal development of the hip resulting in a shallow acetabulum (ball and socket join). Due to this shallow acetabulum, the pt experiences subluxation/ dislocation of the hip from the femur.
What are common s/s of DDH?
-asymmetrical gluteal folds; this is known as the GALEAZZI SIGN
-positive ORTOLANI TEST- palpable clunk when knees bent 90 degrees are then abducted outwards
-positive BARLOW TEST- hip is popped out of socket with knees bent 90 degrees and adducted towards center
What is treatment for DDH and is treatment time sensitive?
**Treatment for DDH has best results when started within first 2 months of life.
Pavlik Harnesses are used for pts less than 6 months of age and worn continuously for 3-5 months.
Spica Casts are used for older children or for children less than 6 months that did not have effective results with the Pavlik Harness.
–Traction may be used for kids with DDH if surgery is indicated. Similar to a spinal fusion, traction would be used pre-operatively to make any possible corrections that way first.
