dental splinting Flashcards
dental splint
1) appliance to stabilize mobile teeth
2) protects them from periodontal injury and provide climate for repair and stabilization in a functional occlusal relationship
3) masticatory comfort and function
indications
1) moderate to advanced mobilities that cannot be treated by other means
- occlusal adjustment
2) increasing mobility
3) prevention of migration and supraeruption
4) orthodontic retention
5) patient comfort
6) prior to SRP and surgery
7) acute trauma stabilization (evulsion of healthy teeth, needs RCT)
8) root removal with a retained crown
ideal splint
1) hygienic
2) stable, simple ,durable
3) no interference with speech
4) no interference with dental treatment
5) non irritating
6) esthetic
contraindications
1) poor oral hygiene
2) compromised esthetics
3) insufficient remaining teeth
4) uncorrectable occlusal interference
5) difficult post splint plaque control
- show clinical crowns
intracoronal
1) tooth structure removed (irreversible)
extracoronal
1) place outside the tooth
2) can be removed
fixed vs removable
1) fixed in place
- not removable by the patient
2) removable
- holly retainers, invisalign
types of splints
1) temporary
2) provisional
3) permanent
temporary splints
1) short term 6 months
2) composite usually
3) types
- composite and wire (A-splint_
- acid etch bonded
- wire ligation
a-splint
1) intracoronal splint
2) removal of tooth required
3) wire material placed in class IIIs interproximally
- composites are placed over
4) no interference occlusally
acid etch bonded splint
1) gold standard
2) composite with or without orthodontic wire
2) composite with reinforced fiber (ribbond)
wire ligation splint
1) typically only for emergency
- trauma
2)materials are easily available
provisional splints
1) several months to years
2) acrylic or metal acrylic
- durable
3) requires preparations of teeth underneath
4) may be used for pending perio surgery or something that needs stabilization
permanent splints
1) permanent
2) fold, porcelain, or other material
cast continuous clasp
1) VA hospitals may use it
2) not as nice of material
3) wire frame is around the facial and palatal surfaces
night guard ana clear aligners
1) invisalign and other night guards
2) usually an upper and lower
3) prevents tooth wear
how to do it
1) measure teeth and cut length of ribbon
2) use tin foil template
- tuck interproximally
3) prepare with pumice or microetch
- then a acid etch
4) pain bonding adhesive resin on tooth surfaces and wet ribbond with resin
- do not forget to wet the ribbond!
5) addapt ribbon to the teeth
- compress into flowable composite1 layer
- tuck interproximally
6) remove excess composite
7) CLEAR GINGIVAL EMBRASURE!
8) light cure it
9) smoothing layer if flowable
- do not get it interproximally
10) adjust with composite bur and check occlusion
tpdau
1) bond adhesive resin to stone teeth then cure with one cycle
2) then gently wet the ribbond
3) squirt small strip of flowable on the mid to upper 1/3 of teeth
- stay clear of embrasures
adapt ribbon to teeth
1) compress ribbon into flowable composite layer
2) tuck interproximally
4) no composite in embrasures
4) cure it
5) smoothing layer of flowable
6) make sure ribbond is completely covered
7) smooth with composite finishing bur
