what fractures are indications for extraction?
- old, complicated
- root involvement
- some fresh complicated
What is the treatment for fresh uncomplicated fractures?
seal exposed dentin with resorative material (glass ionomer or light-cured epoxy)
what is the treatment for fresh complicated fractures with no root involvement?
vital pulp therapy
owners must be willing to come back very periodically for radiographs and they must be aware of potential failure and subsequent need for extraction.
Describe how to perform vital pulp therapy.
- professional dental cleaning
- pumice polish
- rinse mouth with chlorhexidine
- perioperative antibiotics
- remove 5-7 mm pulp with round bur
- control hemorrhage with paper point
- apply pulp dressing (trioxide aggregate)
- Add intermediate layer (glass ionomer)
- finish with light cured restorative
- take post-op xrays
- take follow up xrays in 3m, 12m, then yearly.
