Q

What is gingivitis(Gum Disease)

A

This is often also referred to as Gum Disease.
When normal oral flora is combined with food, this forms what we call plaque. This is not a problem when patients regularly clean their teeth, as they remove the plaque and it does not form a hard scaly substance.

Q

State when Gum disease becomes a problem?

A

The problem occurs when patients have poor oral hygiene, so they do not clean their teeth regularly or properly. This results in irritation of the gums, the gums become inflamed and sore and the surface of the tooth starts to break down leading to decay.

Q

Explain how dental abscess is formed

A

Dental abscesses form when normal oral flora combines with food and drink, particularly those high in carbohydrates.
The bacteria within the plaque will convert the carbohydrates into energy and the acids that are produced will lead to tooth decay.
The bacteria will then spread to the pulp of the tooth causing a dental abscess

A
  • Penicillin
  • Amoxicillin
  • Co-amoxiclav
  • Metronidazole
17
Q

Why does metronidazole need to be prescribed during the treatment of dental abscess?

A

Metronidazole - 400mg TDS for five days (as an alternative for patients with a penicillin allergy). Can also be added in severe cases (infection is spreading, lymph node involvement or systemic signs)

18
Q

What treatment can a pharmacist offer a patient with dental abscess?

A

• Ibuprofen
• Paracetamol (if the patient is unable to take
ibuprofen
• Codeine (not on DPF)
• Diclofenac – however, this is now contraindicated in patients with ischaemic heart disease, cerebrovascular disease, peripheral arterial disease and heart failure.

19
Q

Additional advise

A

. Having the tooth extracted as soon as the infection
and swelling has resolved.
• She should be advised to clean her teeth twice a
day, every day
• visit her dentist on a regular basis. Good oral
hygiene is important.
• The patient should also be advised that oral problems can lead to cardiac complications
