Day 4 - Cranial Nerves Flashcards
Damage to the olfactory nerve will cause what? What about to the inferior temporal lobe?
Ipsilateral loss of smell
Damaged to inf. temporal lobe can cause olfactory hallucinations
How is the optic nerve involved in the pupillary light reflex?
Light is sensed in the optic nerve and is sent to the pretectal nucleus
Fibers from pretectual nucleus project to Edinger-Westphal nucleus of CN III (oculomotor). CNIII innervates bilateral musculature for pupillary constriction (so both pupils will constrict even if light is just in one eye).
Why does oculomotor nerve palsy cause depression and lateral abduction of the eye?
Because all of the eye muscles are weakened EXCEPT the muscles innervated by trochlear and abducens nerves. Therefore, the eye moves in the directions of strength, which are lateral and inferior.
List all of the sx. of oculomotor nerve palsy
- Lateral and inferior strabismus
- Diplopia (double vision)
- Ptosis (eyelid droop)
- Mydriasis (pupil dilated)
- No lens accomodation
The oculomotor nucleus receives axons from a number of structures - list them here
- Oculomotor nerve
- Superior colliculus
- Medial longitudinal fasciculus (connects nuclei of CN III, IV, VI, VIII)
What is the impact of the trochlear nerve fibers crossing prior to exiting the brainstem?
No clinical relevence.
Name the 3 branches of the Trigeminal nerve (CN V)
- Opthalmic (forehead)
- Maxillary (mid face)
- Mandibular (jaw, lower face)
Which brainstem nucleus associated with the Trigeminal nerve innervates the muscles of mastication?
Trigeminal motor nucleus
Which nucleus associated with Trigeminal nerve receives sensory information (touch, pressure, vibration)?
Chief sensory nucleus
Which nucleus associated with the trigeminal nerve receives sensory info related to pain and temperature?
Spinal Trigeminal nucleus
Abducens nucleus receives axons another structure (aside from the abducens nerve) - what is it?
Medial longitudinal fasciculus
List the exceptions to contralateral UMN innervation in the cranial nerves
CN VII (facial) - contralateral UMN innervation to lower face CN X (vagus) - contralateral innervation to palate/uvula CN XI (accessory) - contralateral innervation to trapezius (ips. to SCM) CN XII (hypoglossal) - contralateral innervation of geneoglossus muscle
What does the geneoglossus muscle do?
Depresses and protrudes tongue (bilateral contraction)
Deviates tongue contralaterally (unilateral contraction)
Which cranial nerves are part of the parasympathetic nervous system?
Facial
Glossopharyngeal
Vagus
Oculomotor
Describe the pathway, nerves, and nuclei involved in the gag reflex
Touch felt at pharynx by afferent fibers of CN IX (glossopharyngeal) -> sensation sent to spinal trigeminal nucleus -> cross midline of brainstem -> go up trigeminal lemniscus -> reach nucleus ambiguus on BOTH sides -> motor response produced by CN X (vagus)
