Who must be notified if OSP employee is involved in veh or property incident? And what is requested?
Contact OPS and ask for tracking number and history of sustained like or similar conduct. This info will dictate how incident will be investigated and documented.
Non preventable crash/damage where troop is not at fault?
Supervisor sends memo up chain of command documenting incident, including crash report, bids, and final cost of repairs. All routed to OPS
Preventable crash or damage and no prior sustained like or similar and employee is eligible for informal corrective action, or preventable but justified (such as TVI)?
Memo up chain with photos and docs. In CONCLUSION document the following:
- incident was preventable
- OPS advised troop had no previous like/similar
- informal corrective action you took; verbal counsel or Letter of Instruction
Standard Personnel investigation forms not needed. Memo will suffice.
Include DMV crash report, related bids, and final cost of repairs if applicable.
Preventable crash/damage and there are prior sustained like or similar conduct, and employee not eligible for informal corrective action. Or incident is due to egregious misconduct:
Supervisor conducts personnel investigation.
No memo as personnel investigation will suffice
