how often should you change a CVAD dressing
7 days or as needed
what are supplies you need to gather
CVAD dressing kit and mask for patient if needed
when preparing for care what should you do
clean work surface and allow to dry open external packaging and apply mask have patient turn head away from site perform HH and glove up inspect site for redness or drainage and palpate
if you apply the mask after gloving this will
contaminate the gloves and the patient
when removing old dressing how do you do that
remove towards the site and avoid touching insertion site during removal
when performing the dressing change what do you do
HH open kit for sterile field sterile glove clean site with alcohol dry apply chlorhexadine dry apply antimicrobial disk position dressing over incision label dressing
when cleaning site with alcohol what motion do you clean
starting at insertion site moving in circular motion outward to a 3 inch diameter (3x)
when applying chlorhexadine what is the recommended way
start at insertion site and use friction back and forth working outward lasting 30 seconds and allow to dry for 2-3 minutes
how often should you change needleless ports
every 96 hours, with blood draws, precipitate in connector, cracks, contamination or removal of needless port
