CVA Treatment (Exam 2) Flashcards
Treatment of CVA: First Focus
Airway
Breathing
Circulation
Might need to provide supportive measures first
GCS less than 8 = intubate
After assessing ABC’s you can then initiate a
Stroke protocol
CVA: After checking ABC’s and initiating stroke protocol, what is our next step?
CT Scan without contrast to exclude hemorrhage
Better than MRI in acute stage (MRI takes to long)
Treatment for Hemorrhagic Stroke
-Reverse Anticoagulation
-Manage Hypertension
-Manage elevated ICP
What drug do we give for ischemic type strokes?
Thrombolytics
Fibrinolytics are used to
remove thrombi after they have formed
Alteplase: Class
Fibrinolytic
Alteplase: MOA
Promotes conversion of plasminogen to plasmin
Alteplase: Uses (indication)
Acute MI
PE
Ischemic Stroke
What should we monitor when giving fibrinolytics
BP - HR (bleeding)
Arrhythmias
Hypotension
What must be done first before administering alteplase?
CT scan to rule out hemorrhage
Health History to Confirm that there is no history of intracranial bleed
Alteplase: Adverse Effects
Increased risk of intracranial bleeding
Alteplase antidote
Aminocaproic Acid (Amicar)
-anti-thrombolytic
-used to stop bleeding
Penumbra Procedure / Mechanical thrombectomy
Procedure that goes in and tries to remove the clot
Thrombectomy

CVA: Consequence Dysphagia
Swallow study for aspiration before giving anything PO
Barium swallow
CVA Consequences: Motor and Function
Patients might neglect or ignore parts of body because they are no longer getting sensory input
Increase risk of falls
Loose function of muscle group
CVA: MSK Consequences
Contralateral Side = Flaccidity
Spasticity = Within 6 weeks
Contractures
CVA Consequences: Visual Disturbances
Contralateral field blindness
Homonymous hemianopia (right or left side of both visual field loss)
CVA Consequences Communction
Aphasia
-difficulty understanding
-difficulty speaking
-difficulty writing
-difficulty reading
Aphasia
Some degree of inability to speak or to comprehend
Dysphasia
Impairment of speech
Dysarthria
Imperfect speech sounds
