anomalous origin of the left coronary artery from the right coronary cusp with a subpulmonic course
•The left coronary artery arises from the right coronary artery and then traverses the interventricular septum underneath the right ventricular infundibulum. This is diagnostic of an anomalous left main coronary artery with a septal/subpulmonic course.
L-transposition = congenitally corrected
L for living
D for dead (D-transposition is lethal unless corrected)
malignant course of anomalous origin of RCA from left coronary cusp
malignant means it is interarterial (between the pulmonary artery and aorta)
pt had a prior MI
left vent pseudoaneurysm
The inferior location, morphology of the left ventricular outpouching, and history of previous myocardial infarct is highly suggestive of a left ventricular pseudoaneurysm.
Left ventricular pseudoaneurysms are contained ruptures of the left ventricular wall that often occur after a myocardial infarct. Characteristically, they are inferior or posterior and have a narrow neck relative to the maximum diameter of the pseudoaneurysm

history of radiofrequency ablation
Iatrogenic pulmonary stenosis
