cryoglobinemic vasculitis Flashcards
who gets
90% of cases are associated with hep C infections. IV drug users are at major risk.
cryoglobins
cold antibodies. they precipitate in the cold but redissolve when warmed.
type I clonality
monoclonal
type I rheumatoid factor?
none.
associated diseases with type I
multiple myeloma, wldenstroms macroglobinemia, lymphoma
symptoms of type I
usually asymptomatic until the antibody concentration causes hyper viscosity syndrome.
type II clonality
monoclonal
type III clonality
polyclonal
Type II and III rheumatoid factors
yes
type II associated and presentation
Lymphoproliferative diseases, rheumatic disease, chronic infection, presents as vasculitis, lower extremity purpura, glomerulonephritis, and peripheral neuropathy.
90% associated with HCV infection
type III association and presentation
Rheumatic diseases and chronic infections. Presents as vasculitis. Lower extremity purpura, glomerulonephritis and peripheral neuropathy. 90% associated with HCV infection
treat
if hyper viscosity is present then plasmapheresis is necessary, with chemotherapy to treat the malignancy. otherwise prednisone cyclophosphamide or both.
