clinical judgment or effective problem-solving.

Frail pt. has urinary incontinence. Skin reddened and beginning to break down. To research
for an evidence based intervention, the nurse formulates the following PICOT:
• P: Patient or Problem- For patients with impaired skin integrity related to urinary
incontinence
I: Intervention- would applying a hydrocolloid dressing help
C: Comparison- as compared with applying a gauze dressing after incontinent episodes
O: Outcome- To prevent or reduce sacral excoriation?
T: Time- After incontinent episodes

- Acquire specific and theoretical knowledge related to diagnoses and management of patients and problems
- Do you want to learn strategies that promote critical thinking
- Do you also want to be able to practice
- Ask for feedback from your team and colleagues. If you have a good relationship it’s easier to ask for feedback. Question things don’t be like a robot
