Withdrawal Syndrome Drugs
Beta blockers, clonidine, BNZ, SSRIs, baclofen
Propylene Glycol Toxicity
Assoc w/ hyperosmolar anion gap metabolic acidosis, renal dysfunction, hemolysis, arrhythmias, and seizure
Drug with High Propylen Glycol Content
Benzo (esp lorazepam), etomidate, NTG, barbs (pheno), phenytoin
Carbamazepine Level
4-12 mcg/mL
Digoxin Level
Chronic HF: 0.5-9 ng/mL; A-fib: 1.5-2 ng/mL; may check a level 4 h after IV or 6 h after enteral dose
Lidocaine Level
1-5 mcg/ml; check if tx last 24h or if pt has LV dysfunction or hepatic impairment
Phenobarbital Level
15-40 mcg/mL
Phenytoin Level
10-25 mcg/mL, 2 hr after IV load then again in 2-3 d, 4 h after IM load, adjust to low albumin an CrCl less than 10
Theophylline Level
5-15 mcg/mL, 24 h after initiation of cont IV
Valproic acid Level
50-100 mcg/mL
