Mechanism of endothelial dysfunction
A
- Endothelial cells are infected. Due to cytokine response and viral infection they may suffer apoptosis.
- Monocyte-macrophages release TNF alpha which promote apoptosis of endothelial cells and AECs
- This compromises lung microvascular bed and alveolar cell capillary barrier integrity
- Results in vascular leakage and alveolar oedema

- Increase in Ang II synthesis
- Increased vascular permeability
- Enhanced lung oedema
- Neutrophil infiltration
- Worsened lung function
