what are corticosteroids
–steroids
–are a group of chemicals secreted by the adrenal cortex and are referred to as adrenal cortical hormones
–they are controller… not a rescue agent
Mode of Action of Corticosteroids
–steroids work by reducing inflammation, swelling, and mucus production in the airways. As a result, the airways are less inflamed and less likely to react to triggers
Indications for Use of Inhaled Corticosteroids
–maintenance, control therapy of chronic persistent asthma
-helps prevent the symptoms of asthma
-COPD
-Intranasal aerosol agent: seasonal and perennial allergic and nonallergic rhinitis
Aerosol Steroids: Asthma
–Early use in asthma:
reduces morbidity and airway hyperresponsiveness
spacer is recommended
–Acute Severe Asthma:
not beneficial, for controlling rather than relieving can help improve lung function
–Inhaled Corticosteroids
low dose preferred for decreased side effects
Aerosol Steroids: COPD
–relieves symptoms and exacerbations
–little or no effect on FEV1
Inhaled Corticosteroids: Beclomethasone Dipropionate
–Trade Name: Qvar
–Method of Admin: is a breath-actuated MDI, primeless canister, does not need to be shaken due to having a primeless canister
Inhaled Corticosteroids: Flunisolide
–Trade Name: AeroSpan
–Method of Admin: MDi manufactured with a spacer attached
Inhaled Corticosteroids: Fluticasone Propionate
–Trade Name and Admin: Flovent HFA MDI
Flovent Diskus DPI
ArmonAir Respiclick DPI: spacer should not be used
Inhaled Corticosteroids: Fluticasone Furoate
–Trade Name: Arnuity Ellipta
–Admin: DPI has better affinity for the glucocorticoid receptor this providing a longer duration action
–Given once daily
Inhaled Corticosteroids: Budesonide
–Trade Name and Admin: Pulmicort Flexhaler DPI
Pulmicort respules SVN: is the inhaled corticosteroid available as an SVN. Can be mixed with other liquid medication
Inhaled Corticosteroids: Mometasone furoate
–Trade Name and Admin: Asmanex Twisthaler, DPI,given once daily
Asmanex HFA, MDI, Twice daily over age 12
Inhaled Corticosteroids: Ciclesonide
–Trade Name: Alvesco MDI
–for ages 12 and older
–decreases the incidence of oral thrush
Comination Corticosteroids: Fluticasone Propionate / Salmeterol
–Trade Name and admin: Advair Diskus, DPI
Advair HFA, MDI
AirDuo Respiclick, DPI, is not used with a spacer/holder chamber
Combination Corticosteroids: Mometasone Furoate / Formterol
–Trade Name and Admin: Dulera, MDI
–increase lung function as a combination drug
–steroid and bronchodilator
Combination Corticosteroids: Budesonide / Formoterol
–Trade Name and Admin: Symbicort, MDI, increase beta-adrenergic receptor transcription
